Cardiac arrest or death during epidural anaesth or peripheral nerve blockade. Acute toxicity effects on the central nervous & CV systems. Ventricular arrhythmia/fibrillation, sudden CV collapse & death w/ high systemic conc of bupivacaine. Patients in poor general condition, w/ partial or complete heart block, advanced liver disease or severe renal dysfunction, treated w/ anti-arrhythmic class III (eg, amiodarone). Impaired CV function. Vasoconstriction may aggravate tissue reactions. Chondrolysis. Not indicated for intra-articular continuous infusion. Hypotension. When administered as intra-articular inj, caution is advised when recent major intra-articular trauma is suspected or extensive raw surfaces w/in the joint have been created by the surgical procedure. Patients w/ severe or untreated HTN, poorly controlled hyperthyroidism, ischemic heart disease, heart block, cerebrovascular insufficiency, advanced diabetes & any other pathological condition that might aggravate the effects of adrenaline. Pregnancy (late stage) & lactation. Elderly.
